WorksafeBC Friday released an initial report into the accident that killed Deadpool 2 stunt woman Joi “SJ” Harris.

Harris died Monday morning after a motorcycle stunt went wrong. According to the report, Harris, who is referred to in the report as the “worker,” had been rehearsing a scene that involved driving a motorcycle out the open doors of a building, across a concrete pad and down a ramp that had been built over three stairs and come to a stop on a landing.

It goes on to say that during the first shoot of the stunt, Harris continued driving past the landing, down a second ramp that was built over the bottom stairs and across the street, striking the curb. Harris was thrown off the motorcycle and propelled through a plate glass window.

Deadline Hollywood, an online entertainment magazine, reported earlier this week that Harris was preforming a stunt by character Domino, a new character in the sequel. She was not wearing a helmet at the time of the crash because her character did not wear one in the scene.

WorkSafeBC is continuing to investigate the incident. The production company is required to conduct a full investigation with a report due by Sept. 13.
